Accessibility Assistance

We provide free aids and services to patients with disabilities to communicate effectively with us, including qualified sign language interpreters and written information in other formats. If you need these services, please contact us in advance so we can arrange them for your appointment.

How to File a Grievance

If you believe you have been discriminated against on the basis of race, color, national origin, age, disability, or sex, you have the right to file a grievance. You may file a grievance with our Civil Rights Coordinator by mail, phone, or email using the contact information below.
